DOSAGE AND ROUTES Adults: PO 5 mg bid, then increase dose to 20 mg bid and then 50 mg bid q4wk if hemoglobin (Hb) is below normal or patient has required a transfusion within the last 8 wk. Maintain current dose if Hb is within normal and patient has not required a transfusion within the last 8 wk
