DOSAGE AND ROUTES Adult/child ≥12 yr: INH 2 INH of mo- metasone furoate 100 mcg/formoterol fumarate 5 mcg/ inhalation or mo- metasone furoate 200 mcg/formoterol fumarate 5 mcg/ INH bid, if no re- sponse after 2 wks, may increase to mometasone furoate 200 cg/for- moterol fumarate 5 mcg/INH bid; max, 2 INH bid of mometasone furoate 200 mcg/formoterol fumarate 5 mcg (800 mcg/20 mcg/day)
