DOSAGE AND ROUTES Child ≥2 yr: body surface area 0.55 m(2) or greater and able to swallow capsule whole: PO 25 mg/m(2) twice daily (ap- proximately every 12 hours) until disease progression or unacceptable toxicity; body surface area 1.9 m(2) or greater, give 50 mg twice daily
