Administer: Topical • For topical use only • Shake well before using, apply to dry hair to cover whole area to be treated, including hair and scalp • Massage into the scalp and hair, leave on for 10 min, then rinse with warm water • For single use only, discard unused portion • Do not flush unused portion down sink or toilet • Wash hands after use • Store at room temperature

Patient and caregiver information: • To wash all inhabitants’ clothing, brushes, bedding; that preventive treat- ment may be required of all persons liv- ing in same house, using lotion to de- crease spread of infection • Do not ingest • Keep out of reach of children • Use on children under the direct su- pervision of an adult, risk of benzyl alco- hol toxicity • Avoid contact with eyes • Wash hands after application • Shampoo hair any time after the treatment • Use only once, do not reapply • Discard unused portion, do not flush contents down sink/toilet
