DOSAGE AND ROUTES • Adult: IV 20 mg/m2 over 2-10 min on days 1, 2, 8, 9, 15, 16, then 12-day rest (days 17-28), then may increase to 27 mg/m2 on days 1, 2, 8, 9, 15, 16 repeated every 28 days • Refer to package insert for dos- age adjustments for treatment-related toxicity
