DOSAGE AND ROUTES Spasticity • Adult: PO 25 mg daily × 7 days, then 25 mg tid × 7 days, then 50 mg tid × 7 days, then 100 mg tid (max 100 mg qid); if no further benefit is observed at the next higher dose, decrease dosage to the previous dose; stop if no benefit seen within 45 days • Child: PO 0.5 mg/kg daily × 7 days, then 0.5 mg/kg tid × 7 days, then 1 mg/kg tid × 7 days, then 2 mg/kg tid; max 100 mg qid, if no further benefit is observed at the next higher dose, decrease dosage to the previous dose; stop if no benefit is seen in 45 days Prevention of malignant hyperthermia • Adult/child: PO 4-8 mg/kg/day in 3-4 divided doses × 1-3 days prior to proce- dure, give last dose 4 hr preop; IV 2.5 mg/kg prior to anesthesia Malignant hyperthermia • Adult/child: IV 1-2.5 mg/kg, may re- peat to total dose of 10 mg/kg; PO 4-8 mg/kg/day in 4 divided doses × 1-3 days Neuroleptic malignant syndrome (unlabeled) • Adult: PO 100-300 mg/day in divided doses; IV 1.25-1.5 mg/kg
