DOSAGE AND ROUTES • Adult and child: SUBCUT: 3 mg/kg/ dose weekly for the first 4 wk, then a maintenance dose of 1.5 mg/kg/dose, weekly, 3 mg/kg/dose weekly every 2 wk, or 6 mg/kg/dose q4wk.
