DOSAGE AND ROUTES Secondary hyperparathyroidism in patients with chronic kidney dis- ease on hemodialysis • Adult: IV 5 mg 3×/wk at the end of he- modialysis treatment, max 15 mg 3×/wk For patients switching from cina- calcet to etelcalcetide • Adult: IV: 5 mg 3×/wk at the end of hemodialysis treatment after discontinu- ing cinacalcet for at least 7 days
