DOSAGE AND ROUTES Malaria: • Adults: PO 1250-mg single dose or 750 mg then 500 mg at 6-12 hours after initial dose • Infants, children, and adolescents 6 mo to 17 yr: PO 20-25 mg/kg/dose (max: 1250 mg/dose) single dose or 15 mg/kg/ dose (max: 750 mg/dose) then 10 mg/ kg/dose (max: 500 mg/dose) at 6-12 hours after initial dose
