DOSAGE AND ROUTES • Adult: PO 20 mg every other day × 3 times per wk (on days 1, 3, 5, 8, 10, and 12) for the first 2 wk of each 21- day cycle. Continue for up to 8 cycles; may give up to another 8 cycles (max of 16 treatment cycles) PO on days 1, 2, 4, 5, 8, 9, 11, and 12; cycles 9-16: 20 mg PO on days 1, 2, 8, and 9. Avoid concomitant use with strong CYP3A4 inducers
