USES: Treatment of chronic hepatitis C virus (HCV) infection without cirrhosis or with compensated cirrhosis (Child-Pugh class A) who have genotype 1, 2, 3, 4, 5, or 6 and have previously been treated with an HCV regimen containing an NS5A inhibitor or who have genotype 1a or 3 infection and have previously been treated with an HCV regimen containing sofosbu- vir without an NS5A inhibitor

DOSAGE AND ROUTES Chronic hepatitis C (without cir- rhosis or with compensated cir- rhosis) Child-Pugh class A • Adult: PO Genotype 1, 2, 3, 4, 5, or 6 (previously treated with a combina- tion therapy with a direct-acting antivi- ral): 1 tablet daily × 12 wk. For those with genotype 3 with prior NS5A in- hibitor failure/cirrhosis, use ribavirin also; genotype 1a (previously treated with combination therapy containing sofosbuvir without an NS5A inhibitor): 1 tablet daily × 12 wk Available forms Tabs 400/100/100 mg
